Prescribed medications can continue to be taken, but please notify us if you have diabetes when scheduling your appointment.
Please bring the following with you to your appointment:
Your dedicated doctor is involved throughout your entire medical evaluation process. When you arrive, your doctor will evaluate your medical questionnaire, review your medical and family history, and perform a comprehensive physical exam. Next, you and your doctor will discuss your customized plan for the day and any additional tests or consultations that may be appropriate.
You will meet with your doctor to review and summarize your test results.
